Dear Parents of Tulia ISD Athletes:

**Before you begin please have your athlete’s food service number available as it is required on each form**

1. UIL Physical & Medical History Form

a. You may print the UIL Physical/Medical History Form on the “DOWNLOAD AND PRINT” tab or request one from an athletic trainer or coach.
b. All PISD student athletes must have an updated physical to be able to participate in athletics.
c. Please do not send Physicals through email! Physical forms should be turned into an athletic trainer or a coach or submitted electronically to the Physical Upload Form.

2. Electronically submit the Tulia ISD Online Physical Athletic Paperwork

a. Parents, you must fill out each form.
b. To access the online forms, select Proceed to Online Forms. You will see a list of all electronic forms in blue.
c. Click on the form name and fill out the information requested.
d. To sign the document, click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature”. If you make a mistake and need to start over, click on the refresh icon next to the signature box.
e. You will receive a confirmation email once the document has been reviewed by the staff.

3. A STUDENT WILL NOT BE CONSIDERED CLEARED FOR ANY ACTIVITIES UNTIL ALL FORMS LISTED ABOVE ARE COMPLETE AND SUBMITTED.
